  The Following Documents must be submitted to the parish office on behalf of the Sponsors.
  A copy of the marriage certificate (if married)
  A copy of a valid ID for both sponsors (Driver's License, Passport, etc.)
  A Letter of Permission from your Pastor verifying the sponsors are active and in good standing at their designated parish. (If the sponsors are not registered at Our Lady Queen of Peace)
